Colombians report little to no confidence in judges, the formal legal system and their rights. And yet they continue to file tutela claims. I conducted research in Colombia between July 2016 and May 2017 to investigate this, interviewing 90 lawyers, judges, government officials and service providers, as well as 93 everyday citizens from various class backgrounds. I also surveyed 310 Colombians who were in the process of filing tutela claims. I concluded that citizens view the tutela as the only mechanism through which they can make claims to things they care about, such as health care. Colombians turn to the courts because they see no other alternative, not because of their robust belief in the courts.

As it turns out, both sets of the headlines are true—in some areas, premiums are going down for a variety of reasons. But in most areas, premiums are also going to be higher than they might otherwise have been without various government decisions. Insurer profitability in the individual market started to become much more widespread in 2017 and 2018. And although profitability is obviously the desired goal for insurance companies, they're not allowed to be too profitable. If their total administrative costs (including all overhead expenses plus profits) exceed 20 percent of the premiums they collect, they have to send rebate checks to their members. This is a provision in the ACA that ensures that health plans spend the majority of our premiums on medical costs, rather than administrative costs and profits.

In 2015, the most common health tutela claims had to do with access to treatments (25.6 percent), medications (17.3 percent), prosthetics (11.4 percent) and specialized doctor’s appointments (11.3 percent). Nearly two-thirds of these claims were about items listed as required benefits — in other words, services that all insured citizens should have had access to no matter what, which indicates that significant challenges to accessing health-care goods and services remain. These statistics have been generally consistent over the past 15 years.

A child may be covered by a parent’s health care plan per Affordable Care Act (ACA) regulations through age 25, regardless of whether the child is a dependent for tax purposes; however, under separate IRS regulations, a parent’s HSA funds cannot be used to reimburse for a child’s health expenses unless the child is claimed as a dependent on the parent’s tax return.
Perhaps the most unconventional idea here is to drop health insurance and join a medical cost sharing group instead. These faith-based expense sharing programs are not insurance. Instead members directly share unforeseen medical expenses. Members make a fixed monthly sharing contribution. The groups have set up different systems to either reimburse members for their expenses or directly pay providers for the eligible expenses other members incur. By paying only for actual expenses and non-profit admin fees, the costs of these programs can be very attractive. This was a very popular option for RVers in 2017. 
Health services research can lead to greater efficiency and equitable delivery of health care interventions, as advanced through the social model of health and disability, which emphasizes the societal changes that can be made to make populations healthier.[22] Results from health services research often form the basis of evidence-based policy in health care systems. Health services research is also aided by initiatives in the field of artificial intelligence for the development of systems of health assessment that are clinically useful, timely, sensitive to change, culturally sensitive, low burden, low cost, built into standard procedures, and involve the patient.[23]
 An important note about Avera Health in South Dakota. Avera Health declined to renew our contract to offer their ACA plans in 2019. When we inquired as to why, we received this reply on 10/18/18: “RVers… I am sorry, but a narrow network product does not fit well for the members. We want to be able to take care of our members when they have an accident or illness. We do not have any providers or facilities outside of SD and NW Iowa, which makes us not the best fit for RVers.” While we agree that their plans are not a great fit for RVers, it seems it should be left to the members to decide if they are willing to risk traveling with their coverage. Nevertheless, it looks like RVers are not welcome to enroll with Avera Health for 2019.

Polycystic ovary syndrome (PCOS) is the most common reproductive endocrine disorder in females with insulin resistance playing a key role in pathogenesis. The objective of this study was to investigate current trends and future implications of multidisciplinary PCOS clinics with inclusion of dietitians. A two-phase, formative investigation on practitioners was conducted through an anonymous survey followed by focus groups. Survey respondents included 261 health care providers from around the world; the majority (59%) representing multidisciplinary teams. Focus group participants included four dietitians, three physicians, a health psychologist and a licensed nutritionist. Primary barriers for future multidisciplinary clinics included: money/resources, insurance reimbursement, and difference of opinions. Potential advantages included: more comprehensive and integrated care, greater convenience/efficiency, and better long-term outcomes. A majority of respondents (89%) stated that dietitians should be ‘involved’ or ‘highly involved’ in treatment. The greatest challenges for dietitians include insurance, limited disease knowledge, and lack of referrals. Most providers agreed that multidisciplinary clinics would lead to a better prognosis. A greater emphasis needs to be placed on educating professionals on the importance of nutrition counseling. Access to educated dietitians is likely the best way to ensure that PCOS patients have access to lifestyle interventions. Full article

That’s great, Accidental FIRE. Always good to be aware of options. As far as Aetna goes, I think that is a great place to get coverage through. I’m looking forward to seeing what the combo with CVS will evolve into (I’m picturing expanded roles for minuteclinics, etc) which may provide better options for quality and efficient care at affordable prices. However, in an ERISA case, that insurance company "doesn't engage in the act of insurance", they just administer it. Therefore, ERISA plans are not subject to state laws. ERISA plans are governed by federal law under the jurisdiction of the US Department of Labor (USDOL). The specific benefits or coverage details are found in the Summary Plan Description (SPD). An appeal must go through the insurance company, then to the Employer's Plan Fiduciary. If still required, the Fiduciary's decision can be brought to the USDOL to review for ERISA compliance, and then file a lawsuit in federal court.

HSA funds are considered to be “triple-tax advantaged.” This means any money put into the HSA account is contributed on a pre-tax basis, any earnings on the investments are not taxed, and any funds withdrawn for qualified medical expenses are not taxed. The HSA is an account owned by the employee, and the employee may choose to use HSA funds in the current plan year or roll the account balance forward to let it grow – even into retirement. And if an employee leaves Vanderbilt, the HSA goes with them.

Prescription drug plans are a form of insurance offered through some health insurance plans. In the U.S., the patient usually pays a copayment and the prescription drug insurance part or all of the balance for drugs covered in the formulary of the plan. Such plans are routinely part of national health insurance programs. For example, in the province of Quebec, Canada, prescription drug insurance is universally required as part of the public health insurance plan, but may be purchased and administered either through private or group plans, or through the public plan.[4]
So it does not benefit insurers to just raise rates and pocket the additional premiums. And when it became clear that the premiums for 2018 had been set too high in many cases, the insurers proposed rate decreases for 2019 (or, in some cases, would have proposed rate decreases if not for the factors described above that are pushing premiums higher than they would otherwise have been for 2019).

The last major takeaway from the new CMS rule is the change to Rate Review. Under the Affordable Care Act, insurance companies had to justify any premium increase of 10% or more, but that number will jump to 15% in 2019. Also, the CMS final rule will get state regulators involved in the Rate Review process, and exempt student health insurance plans from federal Rate Review requirements.

The resulting programme is profession-based: all people working are required to pay a portion of their income to a not-for-profit health insurance fund, which mutualises the risk of illness, and which reimburses medical expenses at varying rates. Children and spouses of insured people are eligible for benefits, as well. Each fund is free to manage its own budget, and used to reimburse medical expenses at the rate it saw fit, however following a number of reforms in recent years, the majority of funds provide the same level of reimbursement and benefits.

Recently (2009) the main representative body of British Medical physicians, the British Medical Association, adopted a policy statement expressing concerns about developments in the health insurance market in the UK. In its Annual Representative Meeting which had been agreed earlier by the Consultants Policy Group (i.e. Senior physicians) stating that the BMA was "extremely concerned that the policies of some private healthcare insurance companies are preventing or restricting patients exercising choice about (i) the consultants who treat them; (ii) the hospital at which they are treated; (iii) making top up payments to cover any gap between the funding provided by their insurance company and the cost of their chosen private treatment." It went in to "call on the BMA to publicise these concerns so that patients are fully informed when making choices about private healthcare insurance."[41] The practice of insurance companies deciding which consultant a patient may see as opposed to GPs or patients is referred to as Open Referral.[42] The NHS offers patients a choice of hospitals and consultants and does not charge for its services.

Deductible: The amount that the insured must pay out-of-pocket before the health insurer pays its share. For example, policy-holders might have to pay a $500 deductible per year, before any of their health care is covered by the health insurer. It may take several doctor's visits or prescription refills before the insured person reaches the deductible and the insurance company starts to pay for care. Furthermore, most policies do not apply co-pays for doctor's visits or prescriptions against your deductible.
No individual applying for health coverage through the individual marketplace will be discouraged from applying for benefits, turned down for coverage or charged more premium because of health status, medical condition, mental illness claims experience, medical history, genetic information or health disability. In addition, no individual will be denied coverage based on race, color, religion, national origin, sex, sexual orientation, marital status, personal appearance, political affiliation or source of income.
