Private Health Insurance Rebate: The government subsidises the premiums for all private health insurance cover, including hospital and ancillary (extras), by 10%, 20% or 30%, depending on age. The Rudd Government announced in May 2009 that as of July 2010, the Rebate would become means-tested, and offered on a sliding scale. Healthcare pain. I have a healthy family and we hardly ever go to the doctor. I cover my family through a regular plan through my work with a $3000 out of pocket max per year. Out of the complete blue sky my daughter had a sudden and major health crisis requiring hospitalization starting a month and half ago, this past December. So I blew through $6000 out of pocket in two months by meeting out of pocket for both 2017 and 2018 for one family member. Still would have to meet up to an extra $3000 out of pocket this year to get coverage of anyone else in the family, and also–icing on this cake–I might change jobs shortly, with a new health plan, re-setting all the deductibles to zero. 🙁 This stuff can really wallop you bad. So if you get a very high deductible CAT plan, just realize that if your health problem stretches across two calendar years, you’re going to pay DOUBLE.
As the year comes to a close, I’m reflecting on the past summer and one of the initiatives Healthcare Ready supported that aimed to promote equity in local emergency management policy. We worked with the Baltimore Office of Sustainability on the 2018 update of their Disaster Preparedness and Planning Project (DP3), a comprehensive plan that fulfills a federal requirement that cities must have an All-Hazards Mitigation Plan.
The Medical Loss Ratio (MLR) was put in place under the Affordable Care Act, with the purpose of ensuring health providers offer value to their members. The Medical Loss Ratio is scored from 0% to 100%, and measures the amount of money from member premiums spent by health insurers on members’ claims rather than overhead costs. For example, if a health insurance company allocates $0.90 of every dollar to cover medical claims, and the remaining $0.10 to cover overhead costs, the MLR score for that insurer would be 90%.

I can’t believe I missed this post on the day it published. I guess I am a fat-fire type. (I hate this term since I hate anyone calling me FAT!). After reading this I think I should work 3 more months than planned. I told my office staff that I was going to retire on My next birthday on 7/1. I will be 61. I pay health insurance quarterly so I would have my current BCBS plan paid up through 9/17. If I worked until September I could keep the current policy and then choose a catastrophic plan for the next 3.5 years. This all sounds good to me. I was expecting to pay $20000 minimum for a bronze ACA plan. They should be releasing details later this year.

This is our preferred HCSM plan from Aliera Healthcare that combines the 63 Minimum Essential Coverage preventive care benefits plan with a HCSM for hospitalization. It is an ACA exempt plan because it covers preventive care (after a 9 month waiting period) at 100% with no out of pocket expense to the member and includes an ACA-exempt HCSM hospitalization plan. It is not two separate bundled plans but is one plan through Aliera. Rates look good, benefits are nationwide, and the application is simple. The Statement of Beliefs is non-evangelical personal rights/liberty oriented, making it appealing to a broader audience than some of the other HCSMs.
Sepsis, a syndrome characterized by systemic inflammation during infection, continues to be one of the most common causes of patient mortality in hospitals across the United States. While standardized treatment protocols have been implemented, a wide variability in clinical outcomes persists across racial groups. Specifically, black and Hispanic populations are frequently associated with higher rates of morbidity and mortality in sepsis compared to the white population. While this is often attributed to systemic bias against minority groups, a growing body of literature has found patient, community, and hospital-based factors to be driving racial differences. In this article, we provide a focused review on some of the factors driving racial disparities in sepsis. We also suggest potential interventions aimed at reducing health disparities in the prevention, early identification, and clinical management of sepsis. Full article
Thanks for the post. My wife and I have achieved FI and are exploring when we can retire (she is only working part time now). My biggest challenge is that I have a chronic leukemia that requires medication for life (fortunately I am in remission but still need to take medicine daily). What surprised me the most when searching for health plans on the exchanges, was the lack of hospitals and doctors in the plans. I live in Houston and none of the major hospitals in the medical center are in the market place plans. So if I quit my job I would loose access to the specialist that I have seen for almost 7 years now. I’ve thought of moving to a different state where the plans have access to specific local specialists (of course who knows if the plans in other states will eventually drop those doctors). But for now I feel a bit stuck in my job if I want to visit the doctor and have access to the medical facility that I am so familiar and comfortable with.
Recently (2009) the main representative body of British Medical physicians, the British Medical Association, adopted a policy statement expressing concerns about developments in the health insurance market in the UK. For example, Colorado’s exchange was already in the process of loading 2018 rates into their system when the Trump Administration announced that CSR funding would not continue. But the exchange had to start over on October 13 with the process of loading the backup rates into the system, which delayed the availability of window shopping.

For example, one very poor woman I interviewed recalled a breathing problem she had several years ago. So why are we hearing that average rates are decreasing? It turns out that average benchmark premiums (as opposed to overall average premiums) in states that use HealthCare.gov are decreasing slightly for 2019. The benchmark plan is defined as the second-lowest-cost silver plan in each area (it's also a term used to describe the basic set of benefits that must be covered in each area, but that's not the definition we're talking about here).
Funding from the equalization pool is distributed to insurance companies for each person they insure under the required policy. However, high-risk individuals get more from the pool, and low-income persons and children under 18 have their insurance paid for entirely. Because of this, insurance companies no longer find insuring high risk individuals an unappealing proposition, avoiding the potential problem of adverse selection.
In 2006, a new system of health insurance came into force in the Netherlands. This new system avoids the two pitfalls of adverse selection and moral hazard associated with traditional forms of health insurance by using a combination of regulation and an insurance equalization pool. Moral hazard is avoided by mandating that insurance companies provide at least one policy which meets a government set minimum standard level of coverage, and all adult residents are obliged by law to purchase this coverage from an insurance company of their choice. All insurance companies receive funds from the equalization pool to help cover the cost of this government-mandated coverage. This pool is run by a regulator which collects salary-based contributions from employers, which make up about 50% of all health care funding, and funding from the government to cover people who cannot afford health care, which makes up an additional 5%.[31]
The effectiveness of regenerated chicken bone char (CBC) in fluoride removal was investigated in the present study. Heat treatment was studied as the regeneration method. Results revealed that the CBC regenerated at 673 K yielded the highest fluoride adsorption capacity, hence, 673 K was the best regenerating temperature. The study continued up to five regeneration cycles at the best regenerating temperature; 673 K. The CBC accounted to 16.1 mg F/g CBC as the total adsorption capacity after five regeneration cycles. The recovery percentage of CBC reduced from 79% at the first regeneration to 4% after five regeneration cycles. The hydroxyapatite structure of CBC was not changed during the fluoride adsorption by five regeneration cycles. The ion exchange incorporated with the chemical precipitation occurred during the fluoride adsorption. The repeated regeneration of CBC is possible and it could be used as a low cost defluoridation technique to minimize the wastage of bone char. Full article
Given the premium increases  ahead for 2019, it’s essential for anyone who is eligible for premium tax credits – or who might be eligible with an income fluctuation later in the year – to enroll through the exchange if and when they have a special enrollment period. Don’t sign up for an off-exchange plan and miss out on the possibility of much more affordable premiums via a tax credit.
